The file bg.do provides all the necessary Stata codes to be able to reproduce all the tables in the paper. The files bg-dataset.csv and bg-dataset.dta contain the final dataset in two different formats. The final dataset contains all the necessary data to be able to reproduce all the results of the paper. All the variables are labelled explicitly, so that all the variables are self-explanatory. Also, to ease tracking of variables, they are ordered in order of appearance in the corresponding tables 1 to 4. The zip file bg-ascii.zip contains bg.do and bg-dataset.csv, both of which are ASCII files in DOS format. Unix/Linux users should use "unzip -a". The zip file bg-dta.zip contains bg-dataset.dta.
