The dataset used in this paper consists of the 1996-2001 waves of the 
Medical Expenditure Panel Survey (MEPS). The sample is restricted to 
only those individuals who are covered by private insurance, aged between 
21 and 64 years, employed but not self-employed. There are 20,460 observations
and 36 variables.

The data file MEPS96_01.txt, which is in DOS format, is zipped in the file
dmt-data.zip. Unix users should use "unzip -a".

The data are saved in column, space delimited format with variables
appearing in the order listed below.  Definitions and summary statistics
of these variables are given in Table 1 of the paper.
