The variables used in the paper were constructed from data taken from the
American Hospital Association annual survey of U.S. hospitals for 1984--1996,
with the exception of the wage and case-mix variables, which were obtained
from the U.S. Health Care Financing Administration. A few observations with
missing or implausible values were deleted prior to the esitmation described
in the paper, leaving a total of 36,963 observations.
